The Navigator Project

A study to test the effectiveness of an enhanced case management intervention for HIV infected inmates leaving the San Francisco jails and reentering the community.

The enhanced intervention, provided by the San Francisco Pretrial Diversion Project, provides in-jail HIV prevention counseling and one year of post-jail release intensive case management provided by case managers and paraprofessional peer navigators.  The case managers in this intervention strive to connect participants to appropriate community services (e.g. medical care, housing, food, ADAP, SSI) to increase the likelihood that clients will be stabilized long-term. The peer navigators accompany participants to appointments and provide general support to optimize use of available services.
